The USASA provides Sports Accident Insurance for its players.  Claim Forms (Eng. and Span.) for injuries are available from your League Director    Instructions for completing the new forms can be seen in the PowerPoint presentation links below:

The USASA now provides free Directors and Officers Insurance for SOASA member leagues.  A summary of the coverage may be seen at:
